If you are using the Google™ Chrome browser (version 8.0.552.215) or higher to view the form on our website, you might find that you are unable to save the filled out form. This is because the deafult Chrome PDF reader that is enabled on the Google™ Chrome browser is not the Adobe® version and will not save the information on the form that has been filled out. If you want to save the filled out information, you either save the form on your computer and open it using Adobe® Reader® software or you need to enable the Adobe® Acrobat® plugin on the Chrome browser.
How to enable Adobe® Reader® plugin in Google™ Chrome
- In the Chrome address bar, type chrome://plugins
- Disable Chrome PDF Viewer.
- Enable the Adobe® Acrobat®/ Adobe® Reader® plugin.
